Final Examination Information Form

NOTE: This form is for courses taken in the Summer 2013 semester only!

Please note:  This form is for students requesting to write Distance Education final exams off-campus.  This does NOT apply to on-campus final exams.

It is assumed that all students will be writing their final exam(s) at the University of Guelph. However, arrangements can be made for alternate locations, listed below, provided the exam form is received by Monday, May 27th, 2013.

Please note that a $30 late fee will apply if:

  • You submit this form after the deadline date as this will result in a change in your exam location
  • You request a change in your exam location after the deadline date.  This includes changing your location back to Guelph if you have requested to write off-campus.

We will make arrangements for you to write your final exam at an educational institution, library or government agency near you. One month prior to the examination period you will be mailed details of your final examination date, time and location based on your selection and completion of this form. Your final examination will not necessarily be scheduled on the same day as the on-campus exam and you must be available to write on any date and time during the scheduled exam period.  Due to exam site availability, you may not receive your preferred exam location and may need to travel outside of your desired city. 

Please submit the completed form by Monday, May 27th, 2013.

Please note that students writing outside of Canada are responsible for paying any exam related fees.

Important Note: Students indicating they are registered with the CSD who require additional exam accommodations must be registered with the CSD for the current semester. 

Exam location changes cannot be made after June 24th, 2013.

Reminder: Not all exam requests may be able to be accommodated.  You may need to travel outside of your preferred city if an acceptable proctor cannot be secured.

If you change your exam location after Monday, May 27th, 2013, a $30.00 fee, payable to the University of Guelph must be returned to our office before the examination period begins.

Exam location changes cannot be made after Monday, June 24th, 2013.
